אתמול במפגש...

חבל שלא הגיעו עוד אנשים, מאחר והעלנו, desert ואני, רעיונות לפורום, בתקווה ליישם את הרעיונות, וכן להשתמש בהם, כבסיס לעשייה בעתיד. אם למישהו מכם יש הצעות ורעיונות, נשמח לשמוע, אתם מוזמנים לכתוב אותם.
 
כי מפגש מארגנים במסודר

יש לי נסיון עם זה.. צריך שיהיה טווח זמן מדויק של ימים שבהם מתוכנן המפגש.. ואז מצמצים לפי מה שאנשים יכולים/לא יכולים(רוב..) ואיפה אנשים יכולים ואז קובעים איפה שאנשים יכולים ושעה והכול יפה:)

הכול נכון

אבל היה ברור שלא ניתן לארגן עד חנוכה. לכן השאלה אם יהיה שינוי בכמות האנשים בשבוע הבא . התשובה לא. לכן החלטתי לארגן מיפגש מיד כדי שמי שיוכל יגיע ואפילו אם אדם אחד או שנים. ועובדה שזהב לבן הגיעה והיה לנו כמעט 3 שעות של שיחה בכיף. ויכלנו לשבת עוד כמה שעות. סיפור חיים של כל אחד מאיתנו, יכול להכיל כמה שעות שיחה ואולי לעשות סרט מזה. דיברנו על כל היחסים של האפילפטים אם הממסד והקהילה, איך לשפר את מעמדנו אצלהם. דיברנו על איך לשפר את הפורום ולהגיע לעוד אנשים. ולכול הסקרנים- דיברנו דיבורים שלא לאינטרנט, כי לזה יש מיפגש.
בטח גם ריכלנו. מותר לכם לקנא. בסוף זהב לבן התלוננה שלא רשמנו, אבל בין כה וכה לא הינו מספרים פה לכם הכל. יאללה אנשים , תבואו בפעם הבאה.

בהחלט יכולתי לשבת ולדבר איתך עוד כמה שעות טובות, נהניתי מהשיחה מאוד.

אפשר להביא לפה ד"ר למבוגרים וגם לילדים ועוד משהו אפשר להביא מידי פעם תגובות של אי"ל מידי פעם ,אבל תשתפי אותנו קצת ברעיונות שהעליתם שם במפגש אם אפשר אני אשמח ,בתודה רוני

אין בעיה שאי"ל יגיבו הבעיה היא

שהם יגיבו לנושא השאלה ולא רק אם ברצונכם תפנו לאי"ל בטלפון xyz ותקבעו פגישה. זה היה נוסח התגובות שלהם בעבר ולכן נפרדו דרכינו. לגבי רופא יש לנו רופאה למבוגרים פעם בשבועים - ד"ר קיפרווסר. היה רופא ילדים והוא נעלם משום מה.

מזל טוב זהב לבן - יש הרבה תקוות

מזל טוב על התפקיד החדש. יש כל כך הרבה מה לעשות שקשה ליפול - אפשר לעשות קצת, הרבה והמון והכל מבורך. לפורום, לאי"ל ולפורום בסורוקה יש סדר יום שונה מעט. זה לא אומר שאי אפשר לאחד כוחות על מטרות משותפות. הכי קל זה להתווכח וכולנו יודעים על מה נחרבה ירושליים... אני בטוח שיש לך רעיונות ובכל זאת אוסיף כמה לערימה... לי שני מישורים נראים הכי חשובים - ראשית, מודעות! מהציבור הרחב ועד הרופאים הלא אפילפטולוגים מגלים בורות שכואבת לכולנו. יש המון דרכים להעלות את המודעות. חלקן פשוטות כגון הפנייה יותר בולטת וחזקה לקישורים עם הסבר אל אפילפסיה (בסרגל למעלה לינק נוסף "מבוא לאפי" שמקשר לאחד ממאמרי המבוא הקיימים). אולי תפוז יסכימו לפרסם בכל הפורומים קיום לינק שכזה וכך המודעות לפורום תעלה. ניתן גם לבקש מרופאים מידע בעברית ולצרף למאמרים. איכשהו אני מצאתי הרבה חומר תוך חיפוש מילות קוד בפורום אך כדאי אולי לארגן הודעות נבחרות / בעלות מידע ולשים אותם בקישור נפרד. המישור השני הוא ארגון לובי ליותר זכויות לציבור האפי הגדול - תרופות, ביטוח לאומי, זכויות לטיפול בלקויות למידה ותופעות לוואי אחרים וכו'. חלק מזה מטופל ע"י אי"ל אך חבירה של הפורום יכולה מאוד לעזור. סה"כ מדובר ב - ת מ י כ ה בציבור האפילפטים וזו מטרת הפורום ... יש אולי אפשרות לפנות במסר לכל מי שאי פעם היה בפורום כדי שימסור לך פרטים ואפשר יהיה להחתים בעתיד על עצומות / מכתבים בעתיד. אני לדוגמא נכנס לפורום רק מדי פעם אך מסרים מגיעים אליי גם לכתובת שאני רואה כל יום. המון בהצלחה, אילןאבא נ.ב. אותי מאוד מפחיד "רפואה משלימה" כנושא עיקרי. המון גורמים עושים קופה על ציבור שמחפש תשובות ואם זה יהיה נושא מרכזי זה עלול להרתיע כאלה שסומכים רק על דברים שנחקרו בצורה רצינית.

לכל חברי פורום HAPPY-לפסיה, 1. כשכתבתי את ההודעה הקודמת לא ראיתי שהיה שרשור ארוך בנושא. 2. חלק מהדברים שהתייחסתי אליהם קיימים (כגון שאלות נפוצות) אבל איכשהו נראה לי שניתן לשפר. לדוגמא, אולי כמה מילות קוד עם לינקים לקישורים / הודעות נבחרות. לינקים כאלה יכולים להיות של נושאים שחוזרים כל פעם - שילוב תרופות, סוגי התקפים, דפלפט, תופעות לוואי, רשיון, מין, תורשה, סיסמאות. זה שונה משאלות נפוצות כי לכל נושא כזה יש מספר הודעות רלוונטיות. 3. מקימי הפורום עשו עבודה נפלאה ומאתיים עמודי הפורום והציבור הפאסיבי הגדול שצופה בהם (יש למישהו נתוני צפייה?) יעיד על כך. שכחתי להגיד תודה חמה לחיים שעזר כשהיה צריך בקישורים מהירים ועוד ועוד. לדינה לא נגיד תודה כי זה יתחיל להישמע כמו הספד ודינה לשמחתנו ממשיכה.... 4. אני מסכים עם דינה שיש מקום למתכונים לעוגות אך יש גם מקום לשאלות חצי-רפואיות (בניגוד לכתוב בהודעת מנהלי הפורום) - יש מאגר ידע גדול בפרורם וקיבלתי כמה תשובות וכמה לינקים ממוקדים לעניינים שהטרידו אותי בנושא.

לדעתי - צריך להגיע למצב שרק "אפילפטולוגים" יטפלו באפילפטים. מאחר ויש יותר מידי שחושבים עצמם מבינים ורק מזיקים. הטיעון המרכזי שלי הוא שעלויות של טיפולים שגוים למדינה הם עצומים. מספיק טיפול לקוי בילד שהופך אותו תלוי ביטוח לאומי לכל החים, מוציא לקופת המדינה בישיר והרבה יותר בעקיף. מיליונים רבים שיחלו להחסך. טיפול אצל רופא מתאים יהיה מאוד זול למדינה. בנושא תרופות, יחסית , המצב טוב, לרוב התרופות, למעשה חוץ מאחת, אין בעיה לקבל. בנושא "רפואה משלימה" - אני שותף לדאגה שלך. אבל פגשתי לא מעט אנשים , הורים, וילדים, שהכוונה בנושא תזונה עזרה להם מאוד, וגם כאלה ששיטות סיניות הפחיתו מאד את מיספר ההתקפים. מספיק לראות את הדיאטה הקטוגנית, שהרפואה דחתה אותה והיום היא כבר מוצאת על ידם, ועדין לא יודעים איך היא עובדת.

שלום DesertVoise, מסכים עם רוב מה שאתה אומר. * גם תרופה אחת חסרה זה הרבה... ובשנים המאוד קרובות צפויות הרשויות האמריקאיות לאשר שורה ארוכה של תרופות נוספות כך שהבעיה תחריף... * רפואה משלימה עוזרת לחלק ממי שמאמין ובמקרים אחרים אולי היה שיפור גם בלי הטיפול. נכון, יש דברים שהרפואה לא יודעת ויש שיטות שנבדקו בצורה מדעית והוכחו כטובות גם אם לא מבינים את פרטי השיטה. לגבי תזונה - זה תמיד טוב... חוסר איזון תזונתי יכול להשפיע על הרבה דברים וכל מיני סוגים של חוסר איזון מאוד משפיעים אל אפילפסיות מסוימות (כגון חוסר שינה). לגבי דיאטה קטוגנית -דווקא כן יודעים המון על המנגנון (אם כי עדיין לא הכל ידוע ולא בדקו עד הסוף נושאים כמו תופעות לוואי ארוכות טווח וכו). למציאת מאמרים מדעיים בנושא חפש ב http://scholar.google.com תחת המילים ketogenic diet mechanism. לחילופין ניתן לבצע חיפוש דומה באתר http://www.ncbi.nlm.nih.gov/PubMed יש די הרבה מאמרים. אם יש מאמר מסוים שאתה רוצה להשיג ואין לך רשיון/אישור לפתוח את הלינק המלא (כמעט תמיד התקציר חופשי אך לעיתים קרובות המאמר לא), שלח לי מסר ואנסה לעזור. שבת שלום, אילןאבא

The ketogenic diet provides nutrition with 1 g/kg protein and 5–10 g of carbohydrate per day, with the remainder of calories (usually 75% of the recommended daily allowance) as long-chain triglycerides.9 Meal plans are carefully tailored by a nutritionist for each individual patient. The ratio of fat to carbohydrate and protein ranges from 2:1 to 4:1, with higher ratios seen as more restrictive and possibly more effective. Meals can be quite palatable, including bacon, eggs, tuna, shrimp, vegetables, mayonnaise, and sausages It is perhaps easiest to give the diet to formula-fed infants and patients fed through a gastrostomy tube, because it can be prepared as a liquid preparation n addition, SHS International has created a pre-prepared, powdered, 4:1 ketogenic diet formula, although this is not available worldwide at this time. In addition, the diet can be used for patients who are not refractory to other treatments, although the time and commitment can be considerable when compared with drug therapy. The ketogenic diet's value for patients not in the typical age range (5–10 years old) has recently been shown. Both infants and adolescents can do well and maintain the diet for long periods. here are several disease states in which the ketogenic diet is not only beneficial, but may be life saving. The two main disorders are glucose transporter protein deficiency (GLUT-1) and pyruvate dehydrogenase deficiency. Other disorders—such as pyruvate-carboxylase deficiency, defects of fatty-acid oxidation, carnitine deficiency, and possibly some of the mitochondrial disorders—are thought to be contraindications to the ketogenic diet.9 One disorder that requires more investigation is infantile spasms. infants who had mostly not responded to drugs such as corticotropin and vigabatrin were treated with the ketogenic diet.30 About half had greater than 90% improvement over 3–12 months, including 88% of those who did not respond to corticotropin. Independent predictors of favourable outcomes included age less than 1 year and exposure to three or fewer anticonvulsants. Long-term outcomes Children on the ketogenic diet at our institution are followed up regularly in the clinic with laboratory studies (lipid profile, electrolytes, anticonvulsant levels, and urine concentrations of calcium and creatinine) every 6 months. Parents are advised to check weights and urine ketones at least twice a week and to maintain frequent email or telephone consultation with our group. At this time we do not recommend routine testing of serum beta hydroxybutyrate, but one study indicated it is a more accurate guide to seizure control.34 Drugs for these patients should only be managed by the centre implementing the diet. The diet's efficacy has been assessed in several studies. The largest series of 150 children showed that after 1 year on the diet 50% had a >50% seizure reduction, and 27% specifically had >90% improvement.26 When followed up for an additional 3–6 years, the benefits lasted even after the diet was discontinued; with 44% having >50% improvement.35 Compared with results of most anticonvulsant drugs, which provide a 30–40% chance of >50% improvement in add-on trials, the diet's efficacy is apparent. Because many patients placed on the ketogenic diet have not responded to more than three anticonvulsants, the efficacy is even more significant. A double-blind, placebo-controlled trial (glucose versus saccharin solution given during the initial fasting period) of the diet for Lennox-Gastaut syndrome with multiple seizure types has been completed and is being analysed. This study will be one of the few to formally study the effects of the ketogenic diet prospectively. The ketogenic diet may have other benefits. The money saved through the use of fewer drugs and revised care can be significant.38 and 39 Behaviour has also been reported to be improved in children with epilepsy both with and without autism. Side-effects There are common, uncommon, and rare side-effects of the ketogenic diet. Common side-effects include lack of weight gain (often planned), acidosis (worse with illness), and constipation. Less common are kidney stones (6%), growth inhibition (more significant at young ages), and hyperlipidaemia. The risk of kidney stones does not seem to be increased by the additional use of acetazolamide, topiramate, or zonisamide—anticonvulsants that independently increase the risk of kidney stones by 2–4%. Children had an increase in their cholesterol, triglyceride, and low-density lipoprotein concentrations from the 75th to the 99th percentiles after 3 months. The increase in cholesterol may be caused by ketogenic-diet induced decrease in apolipoprotein B, the major serum carrier of cholesterol. The changes tended to plateau after 6 months and normalised when the diet was stopped. Adjustments to the diet (eg, increased protein and polyunsaturated fats) can be made in children with significantly high lipid concentrations. The long-term results of these side-effects have not been adequately studied, but few children at our centre have had to discontinue the diet because of them. Rare (case reports only) side-effects include cardiomyopathy, prolonged QT syndrome, vitamin and mineral deficiencies, pancreatitis, basal ganglia injury, and bruising. We have not seen these complications in our 500 patients and do not routinely screen for them. When families feel the diet is not effective any longer, it can be tapered slowly while seizures are monitored.
