where exactly do you apply, who determines whether you're worthy and by which criteria?

i'm not sure for the US

but there are a few ways to go there: you should either be really smart rich have family there or found someone who's willing to hire you i think you apply in the embassy, and i believe that some american embassadors and people whose job is dealing with the immigration determine that you're worthy.
